A Case of Anaphylaxis by Dextromethorphan

Abstract

|
Anaphylaxis is a serious allergic reaction that can lead to death in a short time if not treated properly. A case of anaphylaxis occurred after taking a general cold medicine (Alkafen¢ç) from 54-year old female with previous allergy history. Oral provocation tests were carried out with Alkafen¢ç and anaphylaxis developed. However, the patient tolerated another general cold medicine such as Pan Cold A¢ç as well as pseudoephedrine which were included only in Alkafen¢ç. Therefore, the causative agent of anaphylaxis was confirmed as dextromethorphan¢ç. Dextromethorphan is widely used as a component of common cold medicines in Korea and is believed to be very safe. Anaphylaxis caused by dextromethorphan is extremely rare. This patient was the first case reported in Korea. Clinicians should be aware that dextromethorphan is potential culprit agent of fatal anaphylaxis.
